Clinical Nurse Full Time Department: Nursing Job Status: Full Time FLSA Status: Non-Exempt Position Reports to: Clinic Manager OSHA: High Risk Position Positions Supervised: None ADA: Negotiable Position Travel Required: Minimal (more travel if float) Position Summary The Clinical Nurse works as part of the care team and assists Physicians, Advanced Nurse Practitioners, and Physician Assistants in the provision of primary health care. Mainline Health Systems, Inc. involves the Clinical Nurse in continuous Quality Improvement practices and follows a Patient Centered Medical Home model of care. Work Schedule 8 a.m. to 5:30 p.m., Monday‑Thursday; Friday 8 a.m.‑12:00 p.m. (standard). Alternate schedule options: 8 a.m.‑4:30 p.m., Monday‑Thursday (Eudora & Wilmot); 7 a.m.‑4:30 p.m., Monday‑Thursday (Portland Only); 7 a.m.‑3:30 p.m., Monday‑Friday, School Clinics (except Dermott 7:00 a.m.‑11:00 a.m.). Voluntary extended hours may be required on Friday afternoon and Saturday morning clinics. Essential Functions Prepare electronic record each morning for patient appointments; ensure labs, imaging or referral notes are available; pull paper charts for established patients when needed. Triage patients, including: Allergies Vital signs—BP, PR, R, T, BP on all patients 3+; peak flow on asthma patients. LMP/birth control status; document hysterectomy/post‑menopausal status for females. SaO₂ for all patients. Weight/height/BMI; height/length for children; birth weight, length, head circumference as appropriate. Current immunization record and growth chart updates for children 18 or less. Medication list review; document current prescriptions, OTCs, supplements. Subjective statement for clinic visit. Screenings: anxiety annually; PHQ‑2 monthly or PRN for patients 12+; PHQ‑9 and bipolar screening on positive PHQ‑2. Social history annually per guidelines; tobacco smart form each visit. Diabetes foot exam status; pap and mammogram status; colon cancer screening status for eligible adults. Emergency care: recognize and respond to shortness of breath, chest pain, severe allergic reaction, critically elevated/depressed vitals; contact provider or follow standing orders; manage diabetes or cardiovascular follow‑ups per standing orders; document pain location, onset, and scale 1‑10; record injury date; conduct appropriate screening tests. Assist provider with patient examination and procedures: prepare patient, set up room, collect specimens, label and deliver to laboratory, obtain written consent, assist diagnostic/therapeutic procedures, administer medications, make referrals, and complete necessary paperwork. Maintain facility cleanliness: clean exam tables, chairs; restock supplies; disinfect between patients. Keep required logs current: narcotic log, crash cart log, temperature log, immunization log, etc., with proper documentation and frequency. Administer medications as ordered and document in EMR. Provide health education materials to patients. Accurate record‑keeping: scan reports, maintain consultation reports, x‑ray reports, admission/discharge forms timely. Assist patients with indigent drug programs and government assistance. Participate in monthly clinical staff meetings, daily team huddles, and communicate concerns to the care team. Perform additional assignments assigned by the Clinic Manager, Director of Operations, COO, or CEO. Adhere to HIPAA/Patient Privacy; report occupational exposures and incidents promptly. Maintain PPE and biohazard disposal procedures per policy. Ensure all items within the facility meet safety and quality standards. Reporting Relationships The Clinic Nurse reports to the Clinic Manager, who reports to the COO/Director of Operations. In the Clinic Manager’s absence, the nurse reports to the COO/Director of Operations or assigned administrative staff. Physical Demands May lift up to 50 lbs, stand and walk long periods, reach, bend, lift, and perform tasks requiring manual dexterity. Required PPE includes gloves, mask, gown, eye protection, and shoe covers. Qualifications Current license as Licensed Practical Nurse or Registered Nurse in Arkansas. Experience preferred but not required. Competent with EMR documentation; computer and typing skills. Current Basic Life Support for Healthcare Providers. Ability to lift 50 lbs and stand for long periods. Reading and speaking English. Ability to interact warmly and effectively with patients and providers. Availability to work scheduled hours or request time off with advance notice. Salary Salary negotiated with the Clinic Manager based on the established salary scale for Licensed Practical and Registered Nurses. Bilingual employees receive an additional $0.50 per hour. Status FLSA Non‑Exempt position. OSHA low‑risk position. ADA negotiable position. Safety‑sensitive position under Arkansas Act 593 of 2017.
