We're seeking a motivated and compassionate Social Service Director to lead our social services department and support the emotional and social needs of our residents. This is an opportunity for someone who thrives in a fast-paced, team-centered environment and wants to make a real impact.

What You’ll Do:

• Support residents and families during transitions and throughout care

• Create social histories, assessments, and care plans

• Drive admissions, tours, and discharge planning

• Collaborate with the care team and external agencies

• Keep documentation timely and meaningful

• Be the go-to for social needs, referrals, and resources

What You Bring:

• Bachelor’s in Social Work or related field (required)

• Long-term care and supervisory experience (preferred)

• Strong communication, empathy, and organization skills

• Ability to work independently and productively
