A Physical Therapist specializing in Skilled Nursing Facility (SNF) care is needed in Worcester, Massachusetts. This role involves evaluating and rehabilitating patients with physical functioning disorders and wound care, developing individualized plans of care, and supporting patient recovery. Job Details:
- Facility Location: Worcester, Massachusetts
- Schedule: 8-hour shifts, minimum of 40 hours per week
- Required Qualifications: Massachusetts state license; 1 year of physical therapy experienceLevel II experience preferred; experience in long-term care or rehabilitation settings; new graduates with Level II experience are acceptable
- Preferred Qualifications: Experience in SNF or rehabilitation environments; strong documentation and communication skills
- Additional Information: Responsibilities include screening, evaluating, and treating patients with physical functioning disorders, implementing evidence-based interventions, and documenting all clinical activities in compliance with regulatory standards. Candidates must supervise support staff, participate in team meetings, and support the facilitys quality initiatives.
