Core Responsibilities

Education and Qualifications

• MD or DO from a recognized and accredited medical institution.
• Completion of a certified Family Medicine residency program.

Certifications

• Board Certification in Family Medicine.
• Possess (or be eligible for) a valid Kentucky medical license.
