About the Position

Recent Acute Care experience required. License: Active, unrestricted Registered Nurse (RN) license in the applicable state. Certifications: BLS required. Description: The RN implements professional nursing practice centered on the patient as a whole person. A caring environment focused on healing and wellness is established. Nursing practices are based on evidence and current research, as well as policies and procedures, regulatory standards, and professional guidelines. Clinical decision-making incorporates the four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. Coordinating, ensuring, and providing direct patient care activities on the unit are essential. Provision of nursing rounds, assessments, and treatments is included. Facilitating admission, transfer, and discharge activities is expected. Provision of patient and family-centered education is critical. Care coordination and individualized treatment planning are required. Assessment of patient needs is performed and documented in accordance with RN scope of practice and policies/procedures. Consideration of patient culture, age, and developmental level is necessary. Identification of patient risk factors and presenting complaints is essential. Objective and subjective data is collected. Initial admission assessments are verified if obtained by other members of the healthcare team. A problem list is developed, prioritizing patient needs based on input from the inter-professional team. Actions and interventions relevant to patient culture and age are designed. Desired outcomes are determined in coordination with patients and families in accordance with the medical care plan. Patient care therapies and procedures are fully implemented with empowerment and autonomy within scope of practice. Procedures, treatments, and processes are explained to patients and families. Actions to alleviate patient stress and discomfort are initiated. Changes in patient condition are recognized, responded to, and communicated appropriately. Ongoing assessments and responses to procedures and therapies are conducted and documented. Tasks are delegated appropriately and team members are overseen in the performance of patient care activities. Accuracy and completeness of delegated patient care provided by other team members are monitored. Educational needs of patients are identified and addressed according to their level of comprehension. The patient discharge process is facilitated by executing discharge orders promptly. Input into patient/nurse assignments is provided while considering available staffing and patient acuity. Active participation in unit-based activities helps support organizational values and priorities. A positive group dynamic and healing environment are promoted through words and actions. Nursing excellence is assured in accordance with accepted standards, evidence-based practices, and a caring philosophy. Active participation in quality improvement activities and core measures is expected. A holistic perspective regarding patient care is maintained.
