Job Summary:
Seeking an experienced MSW Social Worker for a travel assignment in a Level I Trauma hospital. This role requires dedicated professionals capable of providing comprehensive psychosocial support and discharge planning in a fast-paced environment.

Responsibilities:

• Assess patients' psychosocial needs and develop individualized care plans.
• Coordinate discharge planning and ensure smooth transition for patients.
• Document clinical interactions accurately using Cerner EMR system.
• Collaborate with healthcare teams to address social determinants of health.
• Perform psychosocial assessments and review advanced directives.
• Deliver exceptional customer service to patients and their families.

Qualifications:

• Possess a Master's Degree in Social Work (MSW).
• Hold a valid Florida or Compact social work license on file.
• Have at least 2 years of recent experience in a Level I Trauma hospital.
• Demonstrate prior travel experience in a similar hospital setting.
• Be proficient in using Cerner EMR and Care Port platforms.
• Possess excellent communication and customer service skills.
