RN Rehab Nurse

Bethesda Health is seeking a compassionate RN Rehab Nurse to join our faith-based, nonprofit senior care team. In this role, you will deliver skilled rehabilitation and nursing care, complete assessments, develop and implement care plans, and collaborate with an interdisciplinary team to promote optimal outcomes for residents. You'll provide person-centered care with a focus on dignity, safety, and comfort, while educating residents and families. Responsibilities Provide comprehensive rehabilitative nursing care to adult patients during the night shift Perform holistic patient assessments and ongoing monitoring of clinical status Develop, implement, and update individualized nursing care plans in collaboration with the interdisciplinary team Administer medications and treatments safely and accurately, following physician orders and hospital protocols Provide wound care, pain management, mobility assistance, and support with activities of daily living Educate patients and families on rehabilitation goals, care plans, self-care techniques, and discharge instructions Document assessments, interventions, and patient responses in the electronic health record in a timely and accurate manner Collaborate with physicians, therapists, and other team members to promote optimal functional outcomes and safe discharge planning Recognize and respond promptly to changes in patient condition, escalating concerns as appropriate Follow infection prevention, fall prevention, and patient safety protocols throughout the unit Assist with patient admissions, discharges, and transfers, ensuring smooth transitions of care Prioritize and organize patient care activities to effectively manage a full patient assignment on night shift Participate in quality improvement initiatives and adhere to evidence-based rehabilitation and nursing practices Support an inclusive, compassionate care environment consistent with Bethesda Health's mission and values Required Skills Rehabilitation nursing care Clinical assessment and critical thinking Medication administration and IV therapy Wound care management Pain assessment and management Mobility assistance and safe patient handling Care planning and care coordination Electronic health record (EHR) documentation Patient and family education Infection prevention and control Fall prevention and patient safety practices Time management and prioritization Interdisciplinary team collaboration Vital signs monitoring and clinical surveillance Post-acute and discharge planning knowledge #J-18808-Ljbffr Bethesda Health Group
