Physical Therapist A minimum of 1 year of experience BLS; State Licensure required. Minimum Education Requirement: BA Degree Homecare Homebase EMR is required (no exceptions) About Facility: Home Health Services – York County Address: 226 Northpark Dr., Rock Hill, SC, 29730 General Information: Approximately how many visits per day? 6 total points per day Minimum Years of Experience required? 1 year Will you accept a first-time traveler? Yes, if seasoned to home health Home Health Therapy Services Radius rule: Traveler’s permanent address must be 50 miles outside of home unit Patient Types/Common Diagnoses: Adults – only Chronic diseases, wound care, IV, pleurx, wound vac Required Licensure & Certifications: Driver's License and proof of Insurance required for all positions PT: BLS; State Licensure required. Minimum Education Requirement: BA Degree OT: BLS; State Licensure required.
