Case Manager

Case Manager – PLL North American Family Institute (NAFI) is accepting resumes for a full-time Case Manager to deliver Parenting with Love and Limits (PLL) to adolescent youth and families in Polk, Highlands, and Hardee Counties, Florida. PLL is an evidence‑based community program that partners with the Florida Department of Juvenile Justice. Compensation: $18.54 per hour. Benefits include Medical, Dental, Vision Insurance, 403(b) Retirement Plan, 3 weeks paid vacation, Paid Sick Leave, 11 paid holidays, and Tuition reimbursement up to $2,500 per fiscal year. Location Lakeland Hills Blvd, 1545 Lakeland Hills Blvd, Lakeland, FL 33805, USA Responsibilities Deliver PLL services to adolescents and families in the specified counties. Transport clients as needed. Maintain accurate electronic health records. Work flexible schedule, including evenings. Qualifications Bachelor’s Degree in a related human service field. Experience working with at-risk adolescents. Valid Florida driver’s license with a clean driving record. Reliable transportation and willingness to drive to multiple sites. Computer proficiency and familiarity with electronic health records. Licenses & Certifications None required beyond a valid driver’s license. Employment Status Full‑time position.
