Able Home Healthcare is looking for a compassionate and dedicated Home Health Physical Therapist Assistant to join our growing team in San Diego, CA. Responsibilities:

• Implement physical therapy treatment plans established by the supervising Physical Therapist
• Provide in-home therapeutic exercises, gait training, and functional mobility interventions
• Monitor and document patient progress, reporting changes in condition to the supervising PT
• Educate patients and family members on home exercise programs and safe mobility techniques
• Coordinate care with the interdisciplinary home health team to ensure continuity of care
• Maintain accurate and timely clinical documentation in compliance with Medicare and agency standards
• Travel to patient homes throughout the San Diego area on a scheduled caseload

Requirements:

• Associate's or Bachelor's degree from an accredited Physical Therapist Assistant program
• Active California PTA license in good standing
• Previous home health or rehabilitation experience preferred
• Strong knowledge of Medicare home health guidelines and documentation requirements
• Excellent communication, time management, and organizational skills
• Reliable transportation, valid driver's license, and auto insurance
• Compassionate, self-motivated, and able to work independently in a field-based setting
