LPN Pediatric Home Care - Mandeville

Overview Hammond LA PCS, part of LHC Group, hires a Home and Community Based Services Licensed Practical Nurse to provide direct nursing care to acutely and chronically ill patients in their homes. The nurse reports to a Registered Nurse or designated supervisor and delivers high‑quality care within the assigned geographic service area. Primary Responsibilities Provide high‑quality clinical services within state‑defined scope of practice and plan of care, coordinating with the healthcare team and following all infection control procedures. Document patient visits thoroughly and completely in the electronic medical record, complying with regulatory and payer requirements. Participate in the agency’s care coordination processes, including timely reporting of changes in patient condition, attending case conferences, and communicating with patients, families, physicians, and other care managers. Required Qualifications Current, unrestricted Licensed Practical Nurse licensure in the state of practice. Current CPR certification. Valid driver’s license with mandatory vehicle insurance and access to reliable transportation. Ability to work in any home setting regardless of client background. State‑Specific Requirement (Louisiana) Current LPN licensure in Louisiana without restrictions. Minimum of one year experience as an LPN. Preferred Qualifications Home care experience. Willingness to work flexible hours as needed. Capability to manage multiple tasks simultaneously. Self‑sufficient, independent work style. Strong communication, writing, and organizational skills. Benefits & Compensation Hourly pay ranges from $22.90 to $34.35, depending on local labor markets, education, experience, and certifications.
