Nursing Case Manager
Sevita
As a Nursing Case Manager at Sevita, you will coordinate and manage person-centered care for individuals receiving home and community-based services. You’ll complete assessments, create and monitor care plans, and collaborate with an interdisciplinary team to ensure safe, effective services.
