Job Summary The Registered Nurse (RN), Medical Surgical Float Pool is a professional practitioner who assesses, manages, directs, and provides nursing care activities during the patient’s hospital stay and coordinates care planning with other disciplines utilizing a patient/customer driven approach in a variety of Medical Surgical units. Must be highly energetic, flexible and motivated to support the success of Saint Louis University Hospital. Job Responsibilities and Requirements Assessment and Patient Care: Perform comprehensive nursing assessment/reassessment. Perform age‑appropriate admission assessment or transfer assessment. Obtain input from family/guardian when appropriate. Accurately and completely document findings. Perform assessment of post‑op / post‑invasive procedure patients. Assess and document education and discharge needs of patient and family on admission and throughout hospitalization. Perform patient reassessment documenting pertinent observations according to the patient plan of care, changes in condition, status and/or diagnosis, response to care, procedures, etc., and standards of care. Establish, coordinate and evaluate a plan of care based on analysis of assessment data, patient diagnosis, lab data, tests, procedures, physician orders, protocols and standards of care and other information as relevant. Intervention and Care Planning: Identify short and long term goals based on patient care needs. Formulate nursing interventions to achieve desired patient outcome. Incorporate disease‑specific evidence‑based practice into nursing care plan and other documentation. Provide and document nursing interventions based on assessed patient needs, plan of care, and changes in patient status. Collaborative Care: Collaborate with appropriate health team members for coordination of daily plan of care for assigned patients. Provide, coordinate and communicate patient care, including accurate handoff communication reports such as bedside shift report, ticket to ride, SBAR, daily huddles, patient care conferences, etc. Administer and document medications accurately according to policies and procedures. Monitor, maintain and document accurate IV fluids, blood, blood products and parenteral nutrition according to policies and procedures. Complete referrals as indicated by assessment data. Request consultation for special needs, equipment, or information for patient and/or family. Document restraint use and associated care thoroughly. Document pain assessment, management, and education thoroughly. Document abuse assessment findings and actions. Clarify and document physician orders accurately, including “read back” for all telephone/verbal orders. Recognize changes in patient’s condition and take appropriate nursing actions, using the chain of command when indicated. Involve family/guardian in care and decision‑making as appropriate. Comply with infection control bundles, fall prevention and skin injury prevention initiatives. Restraint Care: Initiate e‑valuate alternatives to restraint prior to application. Apply restraints consistent with the approved procedure. Monitor and assess patient’s response throughout the restraint period at the appropriate intervals. Provide specified patient care (toileting, skin care, hydration, feeding, etc.) on a timely basis. Consult peers to determine alternatives to restraints and 1:1 observation. Pain Management: Assess patient for presence of pain on admission and during assessments/reassessments. Incorporate patient’s cultural/spiritual beliefs regarding pain into pain management plan. Implement pain management techniques, focusing on prevention rather than treatment. Include patient and/or family members in developing a pain management plan. Select additional pain control methods (massage, repositioning, immobilization, music therapy, etc.) as appropriate. Abuse Assessment: Be aware of abuse recognition criteria and incorporate it into assessments. Report signs of possible abuse/neglect to the physician, Risk Management and Social Work. Takes appropriate action to support patient safety when signs of abuse are noted. Evaluation of Care Effectiveness: Perform and document ongoing evaluation of effectiveness of care based on assessment data, nursing interventions, patient’s response to medications, treatments and procedures. Evaluate and document effectiveness of patient/family education. Evaluate plan of care and modify as indicated. Report significant changes in patient’s clinical parameters promptly. Identify problems, gather pertinent data, suggest solutions, communicate using appropriate lines of authority, and work toward problem resolution. Report variation from care/treatment following occurrence reporting policies and procedures. Specialized Care: Provide specialized care to patients at high risk for injury. Quality and Performance Standards: Adhere to all quality and performance standards, policies, procedures, protocols when implementing clinical and technical aspects of care. Participate in learning experiences that increase professional competence. Demonstrate appropriate technical and cognitive skills for area of practice. Maintain currency in all hospital/unit information, communication, policies and procedures. Attend staff meetings/reviews minutes when absent. Participate in committees, shared governance, work teams in a leadership or membership role. Review hospital/nursing publications and stay current with policies and procedures. Provide assistance and support to peers, co‑workers and other team members. Adapt positively to changes in unplanned workload and job demands. Maintain professional relationships with coworkers in a respectful manner. Professional Development and Performance Improvement: Participate in performance/process improvement processes, data collection, suggest solutions, communicate using appropriate lines of authority and work toward problem resolution. Assist in developing unit based performance improvement and action plans. Lead performance improvement team when requested. Contribute to data collection, nursing research activities, and departmental and hospital performance improvement activities when warranted. Age Appropriate Care: Provide age‑appropriate care to infants, children, adolescents, adults, and geriatric patients. Demonstrate knowledge and skills of normal growth and development for the age groups served. Assess and interpret age‑specific data to identify patient needs. Communicate with families or significant others in decision making related to services. Provide service needed for the age groups routinely served. Cost‑Effective Services: Use resources efficiently. Recommend changes that increase efficiency and minimize waste. Document chargeable supplies and equipment used to provide care. Prioritize responsibilities and complete work on time. Utilize non‑clinical time in a constructive manner. Safety and Infection Control: Practice standard precautions and dispose of hazardous waste per established guidelines. Maintain a safe, clean, comfortable, and therapeutic environment for patients/families/employees. Maintain clutter‑free environment and keep personal conversations/noise to minimum. Adhere to dress code and report risk management concerns using eSRM. Complete annual mandatory requirements: up to date license and certifications, BLS, TB screening, influenza vaccination, ethics training, safety training, information security course, abuse & neglect course, blood‑borne pathogens training, tuition coverage, competency, etc. Job Specifications Minimum Education: Licensed to work as an RN in the state of Missouri. Working Conditions: May be exposed to the risk of blood borne diseases. Exposure to unpleasant elements (accidents, injuries, and illness). Subject to varying and unpredictable situations. Handles emergency and crisis situations. Subject to irregular hours. Physical Requirements: Occasional prolonged standing/walking. Occasionally lifts, positions, pushes and/or transfers patients. Occasional reaching, stooping, bending, kneeling, crouching.
