Nurse Practitioner or Physician Assistant, House Calls - King County, WA Incentives & Work-Life Balance Flexible provider schedules available between 24-40 hours per week Flexible work hours between 8:00 AM and 8:00 PM No call, weekends, or holidays About the Role Optum HouseCalls is seeking Nurse Practitioners and Physician Assistants to conduct annual in-home health assessments for Medicare Advantage and other health plan members. This is a non-prescribing, field-based role focused on preventive care, patient education, care gap closure, and collaboration with primary care providers. Key Responsibilities Conduct comprehensive in-home health assessments Review medical history and medications Perform physical exams, screenings, and point-of-care testing Identify and document diagnoses for care planning Communicate findings and care gaps to primary care providers Educate members on disease management, medications, and treatment compliance Address social determinants of health and connect members with resources Recognize and respond to urgent or emergent situations Required Qualifications NP: ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care PA: NCCPA certification and active state licensure Active, unrestricted license (or ability to obtain by start date) Valid driver's license and reliable transportation Ability to work independently in a variety of home environments Preferred Qualifications 1+ years of clinical experience in family practice, geriatrics, primary care, or home health Experience with Medicare or geriatric populations Proficiency with EMR systems and technology Strong communication and patient education skills Ability to transport equipment up to 30 pounds and navigate stairs during home visits Highlights This role offers excellent work-life balance, flexible scheduling, meaningful one-on-one patient interactions, a non-prescribing care model, and the opportunity to improve health outcomes by delivering preventive care directly in patients' homes.
