SWEET HOME CARE SERVICES

CAREGIVER - DIRECT SUPPORT PROFESSIONAL

Position Type: Full-Time / Part-Time

AVAILABLE SHIFTS

·   Day: 8:00 AM - 4:00 PM - $15.00/hour

·   Evening: 4:00 PM - 12:00 AM - $14.00/hour

·   Overnight: 12:00 AM - 8:00 AM - $13.00/hour

POSITION SUMMARY

We are looking for dependable, compassionate, and responsible caregivers to provide direct support to individuals in their homes. Caregivers will assist with daily living activities, personal care, household tasks, meal preparation, community activities, and individual goals while promoting independence, dignity, and choice.

ESSENTIAL RESPONSIBILITIES

·   Provide safe, respectful, and person-centered care.

·   Assist with activities of daily living and personal care as needed.

·   Assist with meal preparation, housekeeping, and household tasks.

·   Support individuals in working toward their personal goals and increasing independence.

·   Follow each individual's care plan, service plan, and support guidelines.

·   Provide appropriate supervision based on the individual's needs.

·   Complete required daily documentation accurately and on time.

·   Document services and activities according to company and Medicaid requirements.

·   Administer or assist with medications as permitted by training and certification.

·   Communicate changes in the individual's health, needs, behavior, or safety to management.

·   Report incidents and safety concerns according to company procedures.

·   Maintain confidentiality and respect each individual's privacy.

·   Encourage meaningful activities and community participation.

·   Work cooperatively with supervisors, caregivers, and other members of the individual's support team.

REQUIRED QUALIFICATIONS

·   Must be dependable and able to work the scheduled shift.

·   Must hold a current Medication Certification.

·   Must hold current First Aid and CPR certification.

·   Must be willing to submit to and successfully pass a required background check.

·   Must be able to communicate effectively with individuals and team members.

·   Must be able to complete required documentation accurately.

·   Must be willing to complete all required company and agency training.

·   Previous caregiving, direct support, or healthcare experience is preferred but not required.

SCHEDULE

We offer flexible 8-hour shifts:

·   8:00 AM - 4:00 PM - $15.00/hour

·   4:00 PM - 12:00 AM - $14.00/hour

·   12:00 AM - 8:00 AM - $13.00/hour
