Job Summary Social Worker - LCSW at Providence Mission Hospital in Mission Viejo, CA. Full Time. 8‑hour day shifts. Responsibilities Work collaboratively on an interdisciplinary consulting team to support patients and families dealing with serious illness. Provide psychosocial assessment, treatment planning, therapeutic interventions, crisis intervention, resource referral, short‑term counseling, grief support, advance care planning. Facilitate family meetings in collaboration with interdisciplinary team. Assume a leadership role to facilitate achievement of patient goals and desired clinical, financial, and resource outcomes. Take an active role in JC Certification in Palliative Care.
