LVN

The Licensed Vocational Nurse (LVN) provides individualized nursing care to residents in a Skilled Nursing Facility (SNF) environment. Reporting to the Director of Nursing, the LVN collaborates with physicians, nursing staff, residents, families, and other departments to ensure optimal patient outcomes. The role requires strong assessment skills, effective communication, adherence to safety and infection control standards, and the ability to perform nursing procedures in accordance with facility policies. Responsibilities:

• Demonstrate professional behavior and maintain positive working relationships with patients, visitors, and staff.
• Conduct individualized patient assessments and notify charge nurse and physician of abnormal findings.
• Develop and implement individualized patient care plans in collaboration with the healthcare team.
• Administer treatments and medications as ordered by physicians, ensuring accuracy and timeliness.
• Perform nursing procedures such as EKGs, blood draws, IV starts, and emergency resuscitation measures.
• Maintain cleanliness and organization of work areas and patient rooms.
• Provide patient and family education tailored to individual needs.
• Document patient assessments, interventions, and progress accurately and promptly.
• Respond to emergency situations appropriately and follow up as necessary.
• Maintain patient confidentiality and provide care that respects patient diversity and dignity.
• Report patient condition changes during each shift and adhere to safety and infection control standards.
• Assist other nursing departments as requested and participate in orientation and education of new staff.
• Coordinate patient transfers and specimen collections as ordered.
• Participate in staff meetings, annual evaluations, and continuing education programs.
• Ensure call lights are answered promptly and nursing care delegated to CNAs is completed and documented.
• Perform other duties as assigned by nursing management.

• Current California Licensed Vocational Nurse (LVN) license.
• Current Basic Life Support (BLS) CPR certification.
• Good physical health and stamina with ability to perform manual skills required for patient care.
• Strong assessment, problem-solving, and communication skills.
• Ability to work effectively in a fast-paced, multidisciplinary environment.
• Dependable with a history of good attendance.
• Ability to communicate in English both verbally and in writing.

• Experience working in Skilled Nursing Facility or acute care settings.
• Interest in continuing education and professional development.
• Proficiency with Microsoft Office applications and medical record documentation.
• Experience with emergency procedures and equipment use.

• Must wear designated Caribbean Blue scrubs as a uniform requirement.
• Ability to tolerate and manage patients with confused or potentially abusive behavior.
• Willingness to work beyond normal hours and temporarily in other positions as needed.
• Must maintain confidentiality and adhere to professional ethics and facility policies.

• Work hours are determined based on department needs and may include variable shifts.
• Participation in end-of-shift walking rounds at specified times to maintain continuity of care.
• Attendance at scheduled meetings and education sessions is required.
