Position Overview

This position involves working in conjunction with physicians, nursing staff, residents, families, and other departments to provide optimal individualized nursing care in a skilled nursing facility (SNF). All nursing staff are required to wear Caribbean Blue scrubs.

Position Qualifications

Must hold a valid California nursing license and be in good physical and mental health.
Current Basic Life Support (BLS) CPR certification is required.
Ability to perform physical tasks such as walking, bending, lifting, and demonstrating manual dexterity essential for patient care.
Capable of managing and tolerating the behaviors of patients who may be confused or exhibit verbal and physical aggression.
Adaptability in following directions and accepting change is essential.
Strong communication skills in English, both verbal and written, are necessary.
Dependable and reliable with a history of good attendance.
Demonstrates strong assessment skills and resourcefulness in emergency situations.
Interest in ongoing education and self-development is highly valued.
Above-average interpersonal, problem-solving, and communication skills.
Ability to work as a team player across departments while maintaining confidentiality and respect for colleagues.

Position Responsibilities

Perform all assigned tasks accurately and in accordance with established guidelines.
Assist with responsibilities across departments as needed while adhering to the chain of command.
Effectively utilize office machines to perform job functions.
Answer phone calls and assist the public in a professional and friendly manner.
Complete additional duties as assigned.

Specific Competencies

Conduct individualized patient assessments, prioritizing data collection based on immediate patient needs and notifying appropriate personnel of any abnormal findings.
Develop individualized care plans reflecting collaboration with the healthcare team.
Perform treatments as ordered by physicians in a timely and accurate manner.
Accurately conduct assigned tasks (e.g., EKGs, blood draws, IV starts) and maintain a clean and organized work area.
Provide individualized patient and family education as needed.
Follow facility policies regarding medication administration; document and assure medications are correct, in adequate supply, and administered as directed.
Document patient assessment findings, responses to interventions, and progress towards resolution of issues.
Initiate emergency resuscitation measures when necessary by following protocols and notifying the appropriate staff promptly.
Maintain confidentiality regarding patient and family matters.
Deliver care in a respectful and non-discriminatory manner, honoring patient diversity and rights.
Report on patient conditions to appropriate personnel during each shift.
Adhere to safety and infection control standards in providing patient care.
Collaboration with other departments to facilitate patient transfers and procedure coordination.
Support the orientation of new personnel and assist in staff education as required.
Assume professional responsibility for maintaining high standards of patient care and follow medication administration protocols accurately.
Participate in annual CPR training and other relevant educational activities to keep skills current.
Engage in monthly staff meetings and assume a role in ensuring patient care continuity across shifts.
Adhere to safety protocols to prevent infections and accidents, reporting any hazards.
Assist with direct patient care as needed, ensuring efficient response to call lights and proper documentation of nursing care provided by support staff.
