Hospice RN Liaison: Care & Community Partnerships

Gulfside Healthcare Services is seeking an RN Liaison to facilitate referrals to hospice, develop community partnerships, and perform timely assessments for referred patients. The RN will represent GHS professionally and ensure high-quality care in line with the mission. The role requires Florida RN licensure, 2+ years acute care experience (homecare/hospice preferred), CPR certification, and valid auto insurance.
