Occupational Therapist (OT) Santana Hills Healthcare Center | San Jose, CA Join Our Team! Santana Hills Healthcare Center is a 167-bed skilled nursing and rehabilitation facility in central San Jose, providing compassionate, patient-centered care to support recovery and improve quality of life. We are seeking a compassionate and motivated Occupational Therapist (OT) to join our rehabilitation team. The OT will evaluate residents, develop individualized treatment plans, and provide therapy to help residents improve their daily living skills, independence, and functional abilities. New graduates are welcome to apply! Employment Types: Full-Time, Part-Time, PRN, and Per Diem Responsibilities

- Evaluate residents' physical, cognitive, and functional abilities.
- Develop individualized treatment plans and therapy goals.
- Provide therapeutic interventions to improve independence and daily living skills.
- Educate residents and families on therapy techniques and adaptive equipment.
- Monitor resident progress and adjust treatment plans as needed.
- Maintain accurate and timely clinical documentation.
- Collaborate with physicians, nurses, caregivers, and other rehabilitation professionals.
- Promote resident safety, independence, and quality of life.
- Follow facility policies, safety standards, and applicable healthcare regulations.
- Bachelor's or Master's degree in Occupational Therapy from an accredited program.
- Current and active California Occupational Therapist license.
- Strong communication, interpersonal, and organizational skills.
- Ability to work independently and collaboratively with an interdisciplinary team.
- Experience in skilled nursing, rehabilitation, or geriatric care preferred.
- Current CPR certification preferred.
