Primary Care Physician (PCP) Responsibilities

• Accountability for outcomes: The PCP demonstrates accountability for outcomes, provides strong clinical care, and delivers cost‑effective management for each patient in their panel of up to 450 patients. They build trust, influence patient behaviors, and help patients achieve improved health.
• Coaching for health: The PCP acts as a health coach, helping patients set short‑ and long‑term goals, partnering with them to progress toward those goals, and frequently following up for continued improvement.
• Simplifying for action: The PCP simplifies and prioritizes care so that patients can act on behavior change and leads their care teams toward performance goals.

ESSENTIAL JOB DUTIES/RESPONSIBILITIES

The PCP independently provides care for older adult patients with acute and chronic illnesses. They take full accountability for patient care and outcomes, seeking specialist consultation when needed while remaining involved in detailed care. The PCP engages with hospitalists for hospitalized patients, performs assessment, diagnosis, treatment, and education, and coordinates health promotion and documentation for complex chronic needs. They lead a care team (medical assistant, facilitator, coordinator) for office patients and collaborate with transitional care teams for non‑office patients. The PCP has an active role in center management, covers for other providers, and recruits patients and additional providers, performing other duties as directed.

• Self‑care: maintaining physical, mental, social, and spiritual well‑being to provide optimal care.
• Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and other relevant software.
• Willingness to travel locally, nationally for initial training and occasionally regionally or nationally for recruiting or training.
• Fluency in English (verbal and written); other language fluency requirements communicated at application.
• Independent judgment in designing and executing care practices.

PAY RANGE

$221,141 - $315,915 Salary
