Isaac Health offers medical consultations, therapy sessions, and cognitive assessments for the diagnosis and treatment of Alzheimer’s and Dementia Related Diseases (ADRD). Experience with people living with dementia or Alzheimer’s is preferred. Equipped for Remote Work: Able to provide your own personal computer/laptop that meets security and telehealth standards (SOC2). Tech Savvy: You know how to troubleshoot your own system and can help others with issues if they arise. Independent: You thrive in a remote environment! Job Objectives Increase patient engagement and retention through compassionate outreach. Facilitate smooth onboarding processes for new patients. Collaborate with internal teams to ensure a seamless patient experience from initial contact to ongoing care. Performance Metrics Conversion rate of initial inquiries to active patients. Number of new patients onboarded per month. Patient satisfaction scores from feedback surveys. Reduction in patient drop‑off rates during the initial consultation phase. Outreach to patients who have expressed an interest in care to explain Isaac Health's services and offerings. Assist patients with technology and other general questions.
