Physical Therapist (PT) Specialty Home Health

Schedule: 8-hour day shifts only

Assignment Details:

• Guaranteed Hours: Up to 6 patient visits/day; may call off up to one shift per pay period

Requirements:

• License: Required, with acceptance of pending licenses
• Experience: 1 year of skilled home health experience
• Certifications: BCLS
• EMR: Experience with OASIS documentation expected

Roles & Responsibilities:

• Evaluate and assess patient needs to develop appropriate treatment plans
• Implement home exercise programs and provide patient/family education
• Facilitate post-operative rehabilitation for orthopedic and neurological conditions
• Monitor and document patient progress and adjust treatment plans as necessary
• Utilize safe body mechanics and mobility techniques during patient interactions

• Dress Code: Business casual attire (scrubs allowed but not required)
• Parking: Free on site
• Communication: Clinician responsible for own communication device if not provided
