Description Clinical Pharmacist for Providence Home Services Specialty Pharmacy in Tigard, Oregon. Per Diem/Variable Shift The Clinical Pharmacy Specialist for Credena Health is responsible for coordination of medication related services as it pertains to specialty pharmacy. Responsibilities include promotion of evidence-based, safe, effective drug therapy, standardization of medication practices across the continuum, and coordination with drug procurement, inventory management and billing processes. Required Qualifications: Education to meet certification, license or registration requirement. Upon hire: Oregon Pharmacist License. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
