Hospice RN Case Manager — Where Support Meets Impact

Lower patient-to-nurse ratios for meaningful, unrushed hospice care

Strong interdisciplinary team support—collaborative care with social work, chaplaincy, aides, and providers, so you're never practicing alone

Dedicated internal after-hours triage team to support nurses outside of regular business hours

Homecare Homebase (HCHB) EMR for streamlined documentation

Office location: 325 N Corporate Dr Suite 260, Brookfield, WI 53045

Coverage area: Travel throughout the territory is required—primarily Waukesha and Milwaukee counties

Schedule: Full time Monday-Friday 8AM-5PM

Responsibilities

As a RN case manager you will:

Assess/monitor physical, emotional, and psychological needs of patients

Create hospice care plans that align with the patient's wishes and goals

Direct nursing care: administering medications, treatments, and interventions

Provide pain and symptom management

Educate and support the patient's family and caregivers

Collaborate with interdisciplinary team

Maintain accurate and timely documentation

Participate in on-call rotation as required by local branch

Qualifications

Graduate of an accredited nursing school with current licensure to practice in state of operation

One year nursing experience required, 3+ years preferred

Experience in hospice or a similar setting preferred

Knowledge of the hospice philosophy of care

Commitment to clinical and documentation excellence

Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
