Registered Nurse - Hospice

Job Description

Job Summary:
Provide specialized hospice nursing care to terminally ill patients, delivering compassionate support and symptom management. The role requires engaging in complex care planning, patient education, and collaboration with interdisciplinary teams to ensure quality end-of-life care.

Responsibilities:

Administer medications and treatments according to care plans, including IV, subcutaneous, oral, and rectal routes.
Conduct comprehensive assessments of patients' symptoms and respond with appropriate interventions.
Participate in goals of care discussions with patients, families, and healthcare teams to develop personalized care plans.
Document patient interactions, clinical observations, and treatment updates accurately in EMR systems.
Monitor patient progress and adjust care strategies based on critical thinking and clinical judgment.
Educate patients and caregivers on disease progression, symptom management, and end-of-life planning.

Qualifications:

Current BLS certification required.
Minimum of 3 years of community hospice nursing experience.
Proven ability to manage complex symptom protocols and provide end-of-life care.
Excellent documentation and typing skills for narrative-heavy EMR systems.
Strong critical thinking and clinical assessment capabilities.
Ability to initiate sensitive conversations and collaborate effectively with multidisciplinary teams.
