Job Title: Nurse Practitioner / Physician Assistant (Acute Care)
Location: Brockton, MA & Surrounding Areas
Job Type: Full-Time/Part-Time (W2 & 1099 options)
Role Overview
Provide on-site primary care, annual wellness visits, and chronic disease management to patients in Skilled Nursing Facilities and Assisted Living Facilities. You will work independently in a patient-centered model to deliver comprehensive care, order diagnostics, manage treatment plans, and improve patient outcomes.
Key Responsibilities
Perform comprehensive primary care evaluations, medical reconciliations, and annual wellness visits.
Diagnose and manage acute, chronic, and infectious health conditions.
Order, interpret, and evaluate diagnostic testing (blood work, X-rays, etc.).
Prescribe medications and formulate individualized treatment plans.
Educate patients, families, and healthcare proxies on medical conditions and preventive care.
Maintain complete and accurate clinical documentation in medical records.
Work Schedule
4 or 5 weekdays per week.
Flexible weekday scheduling options to support work-life balance.
Qualifications
Active Advanced Practice Provider credentials (NP or PA).
Required: Prior acute care experience as a Nurse Practitioner or Physician Assistant.
Prior clinical experience in Skilled Nursing Facilities, Assisted Living, or Primary Care settings preferred.
Strong diagnostic, prescriptive, and patient education skills.
