Responsibilities
- Administer medications per physician orders and facility protocols.
- Monitor and document patient vital signs, symptoms, and changes in condition.
- Provide direct patient care, including activities of daily living and routine nursing tasks.
- Collaborate with supervising RN and care team to implement and adjust care plans.
- Ensure accurate charting, timely documentation, and incident reporting.
- Maintain safety, infection control, and patient-centered, respectful care.

Requirements
- Active Licensed Practical Nurse license in New Hampshire, in good standing.
- Completion of an approved practical nursing program.
- Experience in long-term care or skilled nursing facility preferred.
- Strong communication and documentation skills; ability to function in a team.
- Ability to work in a fast-paced long-term care setting and follow physician orders.
- Willingness to travel and work as assigned.
