Job Highlights Job location: Marshfield Clinic Qualifications High School graduate Post‑secondary training in human services or related field/experience working with people with disabilities may be substituted Two (2) years of work experience Excellent communication skills (verbal and written) Responsibilities Provide case management services and maintain a caseload of Long Term Support Clients supporting them in a variety of job sites Possess awareness of realistic work expectations regarding quality of work and appropriate behavior Ability to effectively communicate in order to act as a liaison between employers and employment services staff as well as individuals care team (funding sources, employers, families/caregivers) Ability to work independently and creatively problem‑solve as needs arise for individuals Reliable vehicle (unless determined exempt with HR) This position requires use of a company email address which requires an application to be downloaded on your personal cell phone for multi‑factor authentication (MFA) to ensure cyber security.
