Responsibilities:

• Provide direct patient care under the supervision of a registered nurse, including administering medications, performing vital signs, and assisting with activities of daily living (ADLs)

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Monitor and record patient conditions, reporting any changes or concerns to the nursing staff and healthcare provider

• Communicate effectively with patients, families, and colleagues to ensure seamless transitions and continuity of care

• Participate in quality improvement initiatives and maintain accurate records and documentation

Requirements:

• Current LPN license in the state of Michigan

• Basic Life Support (BLS) certification

• Ability to work effectively in a fast-paced environment and prioritize tasks accordingly

• Excellent communication and interpersonal skills

• Strong attention to detail and accuracy in record-keeping and documentation
