Medicaid Care Access Navigator
Heart Of Ohio Family Health Centers
Heart Of Ohio Family Health Centers in Columbus, OH is seeking a Patient Navigator to engage Medicaid patients and improve access to care. You will contact patients who miss visits, schedule appointments, and support them with transportation or insurance needs. The role is full-time, on-site, with a Monday–Friday schedule and standard business hours.
