Registered Nurse (RN) Clinical Manager, Home Health

Baptist Health Home Care of KY, LLC located in Louisville is seeking a Clinical Manager - Registered Nurse (RN) to lead our home health services. Schedule: Monday - Friday, 8:15am – 5:00pm. How You’ll Make a Difference As a Clinical Manager, you will ensure high-quality, safe delivery of home health care and positively affect the lives of our patients. We Offer Medical, dental, and vision insurance (flexible options) Paid time off 401(k) with company match Paid life & disability insurance Employee Assistance Program How You’ll Work Operations – Ensure start‑of‑care compliance, manage outcome monitors, participate in on‑call rotations, staff after‑hours, and address patient issues. Process & Complaint Workflow – Report staffing trends, neglect, abuse or theft to the Administrator for resolution. Patient Care – Complete patient visits and assist with any patient issues. Budget – Manage costs, monitor supply expenses, overtime, staffing bonuses, and visits per episode. Hiring & Training – Select, onboard staff and identify education needs. Collaboration – Work with intake to provide daily direction, manage referral volume, and maintain relationships with physicians, hospitals, skilled facilities, assisted living, and families. Supervision – Oversee liaison and clinical staff, ensure timely charting, manage on‑call, PTO, call‑offs, and weekly productivity reviews. Minimize Patient Risk – Ensure quality and safe delivery of home health services. Policies – Review documentation and maintain knowledge of Medicare, Federal, insurance, company and hospital guidelines.
