Social Worker

Job Description Key Responsibilities Conduct comprehensive psychosocial and mental health assessments. Develop, implement, and monitor individualized treatment plans. Provide individual and group counseling services. Perform crisis assessments, triage, and appropriate interventions. Track client progress and adjust treatment plans as needed. Coordinate care and make referrals to healthcare providers and community resources. Maintain accurate, timely, and confidential clinical documentation. Collaborate with interdisciplinary teams to provide integrated healthcare services. Support clients' overall mental health and wellness through ongoing care and education. Qualifications Master's degree in Social Work (MSW) from an accredited program. Active New York Licensed Clinical Social Worker (LCSW) license. 1–2 years of experience providing clinical services in the mental health field. Bilingual (English/Spanish) is a plus. Why Join Us?
