Work Environment

This role is primarily onsite, delivering care in the homes of our patients, which presents unique opportunities and environments for rehabilitation. The structured work schedule from Monday to Friday, 9:00 AM – 5:00 PM, provides you with the predictable hours needed to balance your professional and personal life, allowing you to dedicate your focus to patient care without interruptions.

Key Responsibilities

• Evaluate and assess patient conditions, establishing personalized physical therapy plans that address individual goals and needs.
• Provide direct treatment for an average of 7 patients per day, utilizing various therapy modalities and techniques.
• Guide patients through tailored mobility, strengthening, and flexibility exercises designed to enhance their physical capabilities and overall quality of life.
• Deliver compassionate communication, reassuring both patients and their families as they navigate the rehabilitation process.
• Maintain accurate and timely documentation of patient progress and outcomes, collaborating closely with other healthcare professionals to optimize care.
• Engage in ongoing education and training to stay abreast of the latest advancements and best practices in physical therapy.

Patient Population / Client Focus

In this role, you will work primarily with patients recovering from surgery, injury, or debilitating conditions that limit their mobility. Your expertise and support will be crucial in guiding them back to functional independence. The diverse backgrounds of our patients, ranging from elderly individuals to younger adults requiring rehabilitation, necessitate a compassionate and adaptable approach to care.

Qualifications

• A current Physical Therapist license issued by the New York State Department of Education.
• A Bachelor's degree in Physical Therapy, with a preference for candidates holding a Master's or Doctorate degree.
• A minimum of two years of experience providing physical therapy services in a clinical environment.

Preferred Experience

• Experience working in a home care setting is highly desirable, as it equips you with the understanding needed to provide effective therapy in patients' living environments.
• Familiarity with the technologies and tools utilized in home care therapy, including therapeutic equipment and documentation software, is a plus.

Compensation and Benefits
