Your Role & Impact As a Physician Assistant or Nurse Practitioner, you will provide primary health care services under the direction of the practice physician, working as part of a collaborative care team within an integrated biopsychosocial model. Deliver high-quality, patient-centered primary care Build lasting relationships with patients and families Support the management of complex chronic disease populations Collaborate with behavioral health and ancillary services Participate in practice improvement initiatives Serve as a trusted healthcare advocate within the community Collaborative, Whole-Person Care At High Rock Internal Medicine, you will practice in a model designed to support both providers and patients, with integrated on-site behavioral health, radiology, and ultrasound, and team-based care coordination built into how we work every day.
