HouseCalls Advanced Practice Clinician (NP/PA) Marion and Polk Counties, OR $30,000 Sign on Bonus or $40,000 Student Loan Repayment About the Role Optum HouseCalls is seeking a Nurse Practitioner or Physician Assistant to provide annual in-home health assessments for Medicare Advantage and other health plan members. This field-based, non-prescribing role focuses on preventive care, health education, care gap closure, and collaboration with primary care providers to improve patient outcomes. Key Responsibilities Conduct comprehensive in-home health assessments, including medical history reviews, medication reconciliation, physical exams, and screenings Document diagnoses and findings to support care management and treatment planning Communicate assessment results to primary care providers Identify and address care gaps Educate patients on health conditions, medications, and treatment plans Assess social determinants of health and connect patients with appropriate resources Recognize and respond to urgent or emergent health concerns Required Qualifications Nurse Practitioner: Active ANCC or AANP certification (Family, Adult, Geriatric, or Adult-Gerontology Primary Care) Active, unrestricted state license (or ability to obtain prior to start) Active driver’s license and reliable transportation Ability to travel within the assigned territory and perform in-home assessments Preferred Qualifications At least 1 year of clinical experience, preferably in primary care, geriatrics, home health, or related settings Experience working with Medicare or geriatric populations Strong technology and EMR skills Ability to transport equipment and navigate varied home environments Why Consider This Opportunity Independent, field-based practice Focus on preventive care and improving patient health outcomes No prescribing responsibilities Opportunity to build meaningful patient relationships through in-home care Part of Optum’s growing Home & Community Care organization At UnitedHealth Group, our mission is to help people live healthier lives and make the health system work better for everyone.
