Registered Nurse Case Manager

Location Austin State Supported Living Center, 2203 W 35th St, Austin, TX Job Title Registered Nurse Case Manager (Nurse III) – Health & Human Services Commission, Texas Job Summary Responsible for assessment, care and integrated treatment of residents with intellectual and developmental disabilities in state-supported living centers. Works with an interdisciplinary team to provide holistic, high‑quality care and lead nursing interventions, documentation, and quality assurance activities. Key Responsibilities Perform physical assessments and develop health care plans for residents Oversee nursing interventions and monitor health status Maintain accurate documentation in electronic health record system Collaborate with interdisciplinary team (IDT) to coordinate care Attend on‑campus and off‑campus medical appointments as needed Mentor and coach new RN case managers Participate in quality assurance reviews, committees and workgroups Provide treatment to HHS employees injured on the job Analyze trends in health care needs and report findings Essential Job Functions Attend work on a regular basis and on rotating shift schedules, including overtime and weekends Monitor health status, conduct rounds and record findings Provide hands‑on nursing care and guidance for LVNs and direct staff Act as nursing expert on IDT, educate team and LARs about plans and status Update automated systems and tracking documents and communicate information Complete documentation deadlines consistently Review physician orders and medication records for proper administration Attend medical appointments and coordinate continuity of care Participate in competency training and maintain licensure Maintain safety and infection control practices Possibly support disaster response and continuity of operations duties Knowledge, Skills & Abilities (KSAs) Knowledge of nursing techniques, health care laws, diagnoses and treatment patterns Skill in patient care, record preparation and health care regulation interpretation Knowledge of state and federal laws related to State Supported Living Centers Ability to interact with individuals with intellectual/developmental disabilities, families and staff Computer and EHR system proficiency Time‑management and prioritization skills Excellent verbal and written communication Ability to work independently with all staff levels Current CPR certification and ability to perform CPR Knowledge of emergency procedures and equipment Ability to conduct comprehensive physical assessments and recognize abnormal findings Ability to analyze and integrate information to make nursing judgments Licensing & Certification Licensed Registered Nurse in Texas or a state participating in the Nurse Licensure Compact. Initial Screening Criteria & Requirements At least one year of registered nursing experience Experience in assessment and documentation of health status Preferred: Two years in a health agency, hospital, or health‑care facility Preferred: Bachelor’s Degree Preferred: Experience with individuals with intellectual/developmental disabilities (IDD) Benefits 100% paid employee health insurance, defined benefit pension plan, generous time off, learning and development opportunities including free CEUs and Lippincott access.
