About the Position

Case Management Discharge planning Shift: 5x8s Days (Mon-Friday 08:30-17:00) - NO EXCEPTIONS No Call Minimum 4 yrs exp required. NO FIRST TIME TRAVELERS. EDUCATION Graduate from an accredited school of nursing, BSN preferred. EXPERIENCE Two (2) years of health care experience as a Registered Nurse. EPIC EXPERIENCE REQUIRED Acute Hospital experience required LICENSURE/REGISTRATION/CERTIFICATION Licensed to practice as a Registered Nurse in the state of North Carolina. BLS Required ACM/CCM Preferred No locals - 50 mile radius Client Offers! Candidates will be reviewed/interviewed by MGR Must include RTO in submittal in order to be honored at time of offer! Average daily census Weekends: 70 Days: 20 Orientation: 24 hours The purpose of this position is to provide ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs. The overall goal of the position is to enhance the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through the integrating and functions of case management, utilization review and discharge planning. The Case Manager must be highly organized professional with great attention to detail adaptable to frequent change, and compliant with regulatory and departmental guidelines and policies.
