A Physical Therapist is a healthcare professional who assesses patients with physical impairments, develops personalized treatment plans, and provides manual therapy, exercises, and education to help patients regain mobility, manage pain, and improve their overall function, often focusing on injury prevention and lifestyle modifications to maintain optimal health.

Physical Therapist (PT) Locations: Pasco County, Bradenton, Marco Island, Naples, FL
Employment Type: Per Diem or Full-Time

About the Role:

We are seeking a skilled and compassionate Physical Therapist (PT) to join our Home Health team. The ideal candidate will provide high-quality rehabilitative care to patients in the comfort of their homes. This role offers flexibility, competitive compensation, and mileage reimbursement .

Key Responsibilities:

Conduct initial patient evaluations and develop individualized treatment plans
Provide hands-on therapy to improve mobility, strength, and overall function
Educate patients and caregivers on exercises and techniques to support recovery
Maintain accurate and timely documentation in accordance with home health regulations
Collaborate with interdisciplinary teams to ensure optimal patient outcomes
Manage a caseload of 30 points per week

Qualifications:

Licensed Physical Therapist (PT) in the state of Florida
Home Health experience preferred
Proficiency with EMR systems (HomeCare HomeBase, Kinnser WellSky, or similar)
Strong communication and organizational skills
Ability to work independently and travel between patient homes
