• Contract Duration: 15 weeks
• Shift: 8-hour evenings
• Schedule: Every other weekend required
• Guaranteed Hours: 38 hours per week
• Weekly Estimated Pay: $1,700-$1,790
• Openings: 3
• Charting System: PointClickCare (PCC)
• Care Setting: Post-acute, LTC/SNF

Licensed Practical Nurse Requirements

• Active LPN license in Pennsylvania or compact/licensure eligibility as applicable
• At least 1 year of long-term care experience within the last 3 years
• Current BLS certification required
• Prior experience with PointClickCare (PCC) preferred
• Experience measuring wounds required; formal wound staging is not required
• Ability to work evening shifts and every other weekend
• Experience caring for adult and geriatric residents in LTC, skilled rehab, or dementia care
• Professional, dependable, and team-oriented approach to resident care

Responsibilities

• Deliver safe, compassionate nursing care to residents in a large LTC/SNF environment
• Administer medications and treatments accurately and on time
• Monitor blood glucose levels and administer insulin as directed
• Provide IV medications such as antibiotics and diuretics per care plan
• Support residents with tube feedings, wound care, Foley care, trach care, and oxygen management
• Collect labs and specimens, complete assessment charting, and document incident reports
• Assist with activities of daily living (ADLs) and promote resident comfort, safety, and dignity
• Monitor vital signs and communicate changes in condition promptly
• Collaborate with interdisciplinary team members to support quality resident outcomes
• Use PCC charting and follow established facility workflows and procedures
