Hospice Case Manager

Join our compassionate team as an LPN/RN Case Manager! You'll be the heart of hospice care, coordinating care for patients and families during their most important time. At Auburn Crest Hospice our mission is to create a culture of excellence that honors life by empowering employees, patients and communities to live every moment!
This position is part-time weekends, particularly Sunday. Open for PRN throughout the week as well.What You'll Do:
- Manage patients, ensuring they receive appropriate care.
- Complete assessments, create care plans, and document care.
- Collaborate with doctors, social workers, and other team members.
- Provide support and education to families.
- Enjoy flexible scheduling and work-life balance (avoid burnout!).
- Idaho RN/LPN License.
- 2+ years of experience (pref. hospice).
- Strong communication and teamwork skills.
- Valid driver's license & reliable transportation.
- Registered Nurse licensed in the state of employment.
- Minimum of one (1) year nursing experience, preferably in a Home Health/Hospice setting within the past five (5) years is preferred.
- Certification in Hospice and Palliative Nursing a plus but not required.
- Current CPR certification.
- Must have a valid driver's license and auto liability insurance.
- Understanding and acceptance of hospice philosophy.
- Strong clinical, organizational and interpersonal skills.
- Strong written and oral communication skills.
