Position Overview

We are seeking a compassionate, experienced Registered Nurse (RN) to join our hospice team. The RN provides skilled nursing care, coordinates individualized plans of care, and supports patients and families throughout their end-of-life journey.

The ideal candidate is organized, self-directed, clinically competent, and passionate about delivering exceptional hospice care.

Key Responsibilities

Patient Care & Case Management

• Perform comprehensive initial and ongoing patient assessments.

• Develop, implement, and update individualized plans of care.

• Provide skilled nursing care focused on comfort, symptom management, and quality of life.

• Administer medications and treatments according to physician orders.

• Coordinate care for an assigned patient caseload.

• Educate patients and families regarding medications, symptom management, disease progression, and end-of-life care.

• Identify changes in patient condition and communicate promptly with physicians and the interdisciplinary team.

• Support patients and families with emotional, physical, and psychosocial needs.

Communication & Documentation

• Complete accurate, timely clinical documentation.

• Communicate effectively with physicians, caregivers, facilities, and team members.

• Participate in interdisciplinary group (IDG) meetings.

• Coordinate necessary medical equipment, supplies, and services.

• Promote continuity of care and patient safety.

Additional Responsibilities

• Supervise hospice aides and ancillary personnel as assigned.

• Participate in the on-call rotation.

• Participate in quality improvement initiatives.

• Maintain clinical competencies through continuing education and training.

• Follow organizational policies and applicable hospice regulations.

Qualifications

Required:

• Current, unrestricted California RN license.

• Minimum 1 year of nursing experience.

• Minimum 1 year of hospice experience.

• Minimum 1 year of case management experience.

• Valid driver's license, reliable transportation, and automobile insurance.

• Strong clinical assessment, communication, and organizational skills.

• Ability to work independently and manage competing priorities.

• Ability to meet the physical demands of providing direct patient care, including lifting, transferring, bending, and standing.

Preferred:

• Two or more years of nursing experience in hospice, home health, or community-based care.

• Experience coordinating interdisciplinary care and supervising hospice aides.

Schedule

• Employment: Full-time, 40 hours per week

• Hours: Monday–Friday, 8:30 AM–5:00 PM

• Location: In-person, providing care throughout San Diego County

Schedules may be adjusted based on organizational and patient care needs.
