He/she is employed by St. Tammany Health System (STHS) for the purpose of coordinating care of hospitalized patients.The Advanced Practice Registered Nurse -Nurse Practitioner works in collaboration with and under the supervision of the Lead Hospital Medicine physician and in association with members of the Hospitalist team in the evaluation, treatment, and care coordination of patients. A formal collaborative agreement is a requirement.The major area of responsibility for this role is supporting physicians with the admission of patients to the hospital from emergency room or direct admissions.The primary duties of the Nurse Practitioner under the supervision of the Lead Hospital Medicine physician and in association with members of the Hospitalist team include:Examine patients and review medical history prior to admissionPerform and document a H&P that is co-signed by the attending physician within 24 hoursPlacing orders for appropriate diagnostic and treatment modalities as part of the care planSeeking appropriate specialty and sub-specialty consultationsFormalizing the patient handoff process as part of continuity of careEffective communication with patients, families, and colleagues that are part of the care teamMinimum Qualifications:Nurse Practitioner:Minimum Masters Prepared: Curriculum must include didactic in clinical experience in Acute CareCurrent Louisiana Registered Nurse License (RN)Current Advanced Practice Registered Nurse Licensure in Louisiana (APRN)Eligible for limited prescriptive authority in Louisiana as approved by St. Tammany Health SystemPhysician Assistant:Minimum Masters Prepared: Curriculum must include didactic in clinical experience in Acute CareCurrent Physician Assistant Licensure in Louisiana (PA)Eligible for limited prescriptive authority in Louisiana as approved by St. Tammany Health SystemPhysical Demands:Must possess good physical health. Some requirements include but are not limited to standing, sitting or walking for long periods of time. Lifting at least 50 pounds is required.
