Summary: TheCommunity Resource Navigatorwill serve as a primary point of contact for healthcare patients, as well as the communities we serve, in an ambulatory care setting.They will support individuals and families entering the healthcare system andassistwithutilizationand retention of benefits and community resources. They will also focus on helpingeliminatebarriers to all JPCHC services.This position will float to clinics as needed. Essential Duties and Responsibilities :Essential Duties include, but are not limited to the following: Work outreach events during traditional and non-traditional hours and locations Work collaboratively with all the different departments of the JPCHC organizations Meet with self-pay patients to discuss payment options and complete Sliding Fee Scale agreement Maintainsahigh levelof confidentiality while interacting with patients and clients with empathy and respect Research and resolve patient or client issues, including consulting with providers, plan administrators,andinsurance companies Educate members on issues related to using the healthcare system and their specific governmentassistanceplan Maintain HIPAA standards and confidentiality of protected health information Follow up with clients/patients who request in-person government assistance Maintain Food Pantry at designatedlocation,including pantry access, stocking, ordering,and cleaning. Follow through with Tele-Visit appointments (appointments where the consumer is at a separate location from where you arelocated) Assistpatients with applications andfollow-upfor Medicaid programs and Marketplace coverage Assistpatients with Medicare Process income information for the JPCHC Sliding Fee Scale applications Assistpatients with applications for various Prescription Assistance Programs Provide community resources to the underserved, vulnerable populations,and patients Participate in outreach and education programs for the community, such as health fairs, screenings, and other events Assist with Social Determinants of Health where needed Assistwith special projects and other duties as necessary andrequired Perform related duties as assigned Qualifications : To perform this job successfully, an individual must be able toaccomplisheach essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or abilityrequired. Education and/or Experience: High School Diploma or Equivalent required. Two years ofpreviousexperience in outreach navigation preferred Must be willing to travel 50% of the time to different site locations Valid Indiana Driver’s License and proof of auto insurancerequired Certified Indiana Navigator certificationrequiredwithin90 daysof employment Certified Application Counselor certificationrequiredwithin90daysof employment Community Health Worker certificationrequiredwithin 6 months of employment Computer Skills To perform this job successfully, an individual should have proficient computer skills, including Microsoft Office (specifically Outlook,Excel,Word, and PowerPoint) and Adobe Acrobat.
