Care Manager RNCare Manager RN at Providence Queen of the Valley Medical Center in Napa, CA. This position is Full time and will work 8-hour, Day shifts.The RN Case Manager is responsible for carrying out effective case management functions for identified patients by planning, coordinating, negotiating, procuring and managing the care and needs of complex patients and their families throughout the continuum of care. The RN Case Manager assists patients in the utilization of appropriate health care services. Your voice matters here, because we know that to inspire and retain the best people, we must empower them.Required Qualifications:Coursework/Training: Recent completion of a post-graduate level curriculum focusing on case management or:Current clinical experience with 2 years clinical experience in an acute care hospital setting, or1 year current experience in an inpatient or outpatient case management setting.California Registered Nurse License upon hireTwo (2) years case management, discharge planning, or utilization management experience in the acute, sub-acute, home health setting or managed care environment.Preferred Qualifications:Bachelor's Degree: Nursing.
