Develop and implement individualized treatment plans, with mobility progression from acute care through SNF, and discharge planning. Manage 30–45 minute outpatient treatment slots and one-hour initial evaluations; see a daily caseload of 5–15 patients as appropriate. Provide care across ER, ICU, Med/Surg (Acute, Observation, SNF) and occasionally OB; contribute to a hospital-based outpatient PT clinic. Document patient progress, adjust plans of care, and communicate with the care team to optimize outcomes and safety. Support discharge planning through collaboration with physicians, nurses, and therapists, ensuring smooth transitions of care. Requirements Active Hawaii physical therapist license (or eligibility) and ability to obtain license promptly. Demonstrated ability to perform PT evaluations and treatments across inpatient and outpatient settings. Knowledge of discharge planning and mobility progression. Ability to work a 5x8 schedule, including Saturdays as required. Comfortable practicing in diverse clinical settings and coordinating across acute and outpatient services.
