Job Description
Registered Nurse - Case Manager

Schedule

Day shift, 5 days per week, 8 hour shifts (08:00 - 16:30). Required to work every other weekend.

Assignment Details

Start date: ASAP. 2 years of experience required. Floating between like units may be required based on need.


Requirements

• Must have acute care experience
• First-timers accepted with clinical approval
• Charting: Epic
• All requested time off (RTO) must be included at the time of submittal.

Responsibilities

• Manage patient care and coordinate with multidisciplinary teams.
• Perform assessments and develop individualized care plans.
• Facilitate communication between patients, families, and healthcare providers.
• Monitor patient progress and adjust care plans as necessary.
• Ensure adherence to policies and best practices in patient management.
