Senior RN Case Manager - Care Navigator with Sign-On Bonus

OptumCare in Las Vegas seeks an experienced Clinic Case Manager to support high-risk patients with care navigation and education. You will collaborate with physicians, nurses, pharmacists, social workers, and educators to ensure optimal quality of life and safe transitions across care settings.
