Nurse Care Manager

Company that provides compassionate, customized care for seniors and individuals with disabilities across Virginia, DC, and Maryland seeks Nurse Care Manager for hybrid schedule. Hours/Schedule 32-40 hours/ week, this is an hourly position and work can flex up or down each week, but on average we're looking for someone to work 32 to 40 hours a week. Job Type Direct hire Location Requirements Hybrid - onsite work will be with clients at their home and doctors' appointments. Rate $40-47/hour depending on experience, employees receive benefits of PTO, ICHARA Health Benefits and 10 holidays at the 32 hour mark. Job Description The Nurse Care Manager serves as a liaison between the client and their health care providers, utilizing the nursing process as the foundation of practice. This role is responsible for identifying the client's medical, functional, mental, psychosocial, and environmental concerns, and planning interventions to help the client obtain the best quality of life. The Nurse Care Manager works collaboratively with the Client Care Manager to ensure that resources are available to support client needs. Responsibilities Uphold the organization's mission, vision, and values and incorporate applicable ethical principles in all client activities. Maintain the highest standard of confidentiality (HIPAA) in all areas of work, including obtaining yearly HIPAA authorizations from clients or their guardians. Establish and maintain a trusting, therapeutic relationship with clients; recognize each client as a unique individual with their own beliefs and preferences. Serve as a liaison between the client and their health care providers; attend client medical and healthcare provider appointments as appropriate. Assist the client in understanding their medical condition and in making present and future decisions; provide education on medical conditions, tests, treatments, medications, safety, and preventative health. Conduct research to identify relevant standards and guidelines that support or offer alternatives to provider recommendations. Assess client needs through client and family interviews, medical record review, and on-site home visits; continually reassess and modify the care plan as needed. Identify and recommend steps and actions to assist the client; implement and evaluate those steps, modifying as needed. Work collaboratively with the assigned Client Care Manager (CCM) to meet the client's goals. Verify whether recommended care and services are covered by the client's insurance; review Explanation of Benefits (EOBs) to confirm charges are appropriate. Transport or arrange client transportation to appointments as needed. Assist with client housing transitions, including needs assessment, facility entry requirements, and coordination on moving day. Complete and submit Comprehensive Assessment Reports; schedule review meetings with the client and care team within the 10-day timeframe. Enter case notes within 24 hours; maintain clear, concise, comprehensive, and grammatically correct documentation per established format. Enter billable hours accurately and maintain a tracking system for billable time. Upload medical information and update the secured data entry system promptly per established policy. Communicate client updates verbally and in writing within established timeframes; provide periodic written and verbal updates to clients and families on goal progress. Communicate issues and concerns up the chain of command; report actual or potential cases of abuse or negligence to management, the client's guardian, and all relevant authorities. Provide court testimony or deposition addressing care manager efforts and observations, as required. Attend staff meetings and Nurse Care Manager meetings for client updates, in-services, and staff development. Qualifications Current, active nursing license (RN or LPN) in VA, MD, or DC in good standing. Minimum of 10 years of clinical experience. Proficient in the use of a smartphone and Microsoft Office products. Valid driver's license and current car insurance. Must pass civil and criminal background checks. Strong written and verbal communication skills. Self-motivated and comfortable working both independently and in a team environment. Ability to multi-task and manage priorities.
