Registered Nurse- RN Hospice On-Call (Mon-Fri)

Newport News, VA

We are seeking a Hospice RN On-Call Nurse to provide after-hours support for patients in the Newport News, VA region . This position covers on-call services Monday through Friday , with the schedule beginning Monday at 4:30 PM and continuing through the week until Saturday at 8:00 AM .

Pay: $80,000-$90,000/annually

Job Summary: The Hospice RN On-Call Nurse provides after-hours clinical support and on-call services for the Hospice Department, ensuring timely response to patient and family needs. The Hospice RN On-Call schedule begins Monday at 4:30PM and ends on Saturday at 8:00AM . The coverage area includes all locations serviced by the Hospice Department.

This position offers a unique opportunity for an experienced and compassionate hospice nurse seeking a flexible schedule while making a meaningful impact on the care and support of hospice patients and their families.

Responsibilities:

1) Completes an initial and comprehensive assessment of patient and family to determine hospice needs.

2) Provides a complete physical assessment of history of current and previous illness(s) as well as pain and symptom control.

3) Provides professional nursing by utilizing all elements of the nursing process and follows the State Nurse Practice Act.

4) Assesses and evaluates patient's status by:

a. Writing and initiating POC.

b. Regularly re-evaluating patient and family/caregiver needs.

c. Revising the POC as necessary.

5) Initiates the POC and makes necessary revisions as patient status and needs change.

6) Uses health assessment data to determine nursing diagnosis.

7) Develops a PC that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions in concert with the IDT group. Includes patients and family in the planning process.

8) Initiates appropriate preventative and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.

9) Provides health care instructions to the patient/family as appropriate per assessment and plan.

10) Assists the patient in activities of daily living and facilitates the patient's efforts toward self-sufficiency and optional comfort care.

11) Acts as a Case Manager when assigned by the Director of Patient Services/Hospice Director and assumes responsibility to coordinate patient care for assigned caseload.

Additional Duties:

1) Works the Hospice RN On-Call after hours work schedule as defined above.

2) Provides death visits, as necessary.

MEDICAL SPECIALITIES:

• Home Health

• Hospice & Palliative Medicine

QUALIFICATIONS:

• Graduate of an accredited school of nursing.

• Two years' experience of recent acute care experience in an institutional setting and One year of recent experience in a home care setting, preferred.

• Current licensure in State of Virginia, CPR certification and a valid VA Driver's License.

• Prefer bachelor's degree with one year of Hospice experience.

• Management experience not required. Responsible for supervising Hospice aides and Hospice LPN's.

• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills, hospice nursing skills per competency skills checklist.

Ability to commute/relocate:

• Chesapeake Virginia: Reliably commute or planning to relocate before starting work (Required)

Experience:

• Nursing: 1 year (Required)

• Hospice: 2 years (Required)

License/Certification:

• BLS CPR Certification (Required)

• RN (Required)

Job Type: Fee For Service /FFS or PRN

Pay: $80,000.00-$90,000.00/annually
